Why can't the 222 attack ground ?

False. A simple true or false for behavior has not been set which is why it cannot attack ground.

i think maybe it would have clashed with the 221 which had a mg only ? thus needs to be re coded

would an attack ground on a small arm (lmg 42) really be op ?

It would be on HMGs as you could bypass cover.
Instead of targeting a squad you could just attack ground next to it, suppressing it via aoe.

I think you are wrong here, because cover provides suppression recover boost and reduces the amount of suppression dealt to a squad. otherwise squads in cover next to a squad which is target of a HMG would be suppressed really fast, but they aren't.

I will simply lol at anyone expecting such a change to a 2 year old general use infantry.
Equally possible are pershing for USF, IS-3 for soviets or ober nerf for OKW.

20 fuel garbage can also get some extra gun to oneshot mediums for balance sake. The normal q is why at infantry doesnt have AG ability to counter vehicles attacking from visual cover.
