Imo the Prearranged teams create issues (at least in COH2) with creating matches that are imbalanced. The reason for that are:
1) Prearranged teams are treated like new players for the first 10 games inflating the numbers of new players and creating games where experienced players can be matched up against less experienced player that can lead to big elo losses for that later
2) (This based more on assumption) The algorithm searches for Prearranged teams and when it fail to find them immediately expand the criteria to include single players with great Elo difference.
Imo one can fix these issue with some of the following changes:
1) Players are allowed to play with friends in normal ladder but not as "prearranged team"
2) There is separate ladder for teams where players play as team even if not full (for instance a 2 player team in 4vs4 mode)
3) Each player has two separate elo for each mode one as individual and one as a team
Alternatively a player in a prearranged team uses his higher Elo for classification.

Honestly this would be a fantastic addition to help balance matchmaking for team games.
