Make sure you do not have Thumbs.db or any files in your mod folder that CoH2 does not support.
Also, if the decal is the same for every faction, you do not need a texture for every faction. Take a look here for an example:

Then, I am not sure what to tell you. It works fine for me, after building on my end.
My guess is that you are not actually building the mod, or have errors in the Mod Builder preventing you from doing so.
Try using the Archive Viewer on the .sga file in "Documents\My Games\Company of Heroes 2\mods\decals" to see if the preview image is still in info.

I have no idea what that .tar file is, but that is the reason right there (TOC source).
Ah, just realized what it is. Change "Curated" to "Release" in Mod Builder.
